How to Tell Sunspots Apart from Melasma

Sunspots & Pigmentation

  • Discrete spots with well-defined, clear borders
  • Darker in appearance — more superficial in the skin
  • Circular in shape; can occur anywhere on the face or body

  • Requires deep-penetrating laser — IPL is insufficient

What is Post Inflammatory Hyperpigmentation (PIH)?

PIH, also known as Pigmented Scars, occurs when skin is injured through sun overexposure, acne, or aggressive treatments. Inflammation triggers melanocytes (pigment-producing cells) to send melanin to the surface as a protective response.

Common causes include misuse of Retin A, Hydroquinone, home chemical peels, or bleaching creams. Sometimes PIH and melasma exist together on the same face. The good news is both respond to the same treatment protocol.

Melanocytes send melanin to the skin's surface to protect it during inflammation. This pigment can form spots or blotches throughout the face, which can mimic melasma, making diagnosis essential.

PIH is not as deep as melasma, so it responds faster to the Spectral Q-Switch laser. We generally recommend 6 weeks of brightening cream preparation before starting laser treatment.

Treatment protocol: Minimum 12 Spectral Laser sessions with cooling measures applied before, during, and after every treatment to prevent further inflammation.

What Triggers Hyperpigmentation

Genetics, pregnancy, menopause, thyroid conditions, and hormonal contraceptives are primary drivers. These internal factors determine your baseline susceptibility to melasma.

Sunlight is the most significant activator of hyperpigmentation. Even brief exposure can re-stimulate melanocytes. Consistent broad-spectrum SPF use is non-negotiable for long-term results.

Dry, inflamed skin is far more susceptible to pigmentation. Over-aggressive treatments, misuse of acids, and an inflammatory diet all worsen pigmentation over time.

Stress hormones, poor sleep, fatigue, smoking, and fast food diets all elevate melanocyte activity. Lifestyle optimization — sleep, exercise, antioxidants — is part of our program.

As skin ages, its ability to regulate melanocyte activity decreases. Cumulative sun damage and slower cell turnover make pigmentation more persistent and harder to fade without targeted treatment.

Daily use of cosmeceuticals that suppress melanocyte activation in sunlight is highly effective. We'll prescribe a personalized home-care regimen — including our PMD Trinity Line — to extend and protect your results.
